Information on how to change, update, or correct your personal information with the Universities of Wisconsin can be accessed below.
How to Make Changes
Address
- Review Home Contact Change-Employee (KB 144891) for information on how to update your home address.
- Note: The address you indicate as Primary will be sent to benefit vendors. If you have multiple addresses indicated as primary your home address will be sent to benefit vendors.
Name
- Review Legal name Change-Employee (KB 144831) for information on how to update your legal name. Documentation may be necessary.
- Dependent’s Name: Update your dependent’s name in My Insurance Benefits. To make changes log into Workday (myworkday.wisconsin.edu). Once in Workday, from the Menu in the left go to Personal, then select Benefits and Pay. In the Suggested Links, select My Benefits to access the benefits enrollment system, My Insurance Benefits.
Gender
- Reach out to your human resources contact for information on the process to change or correct gender and for documentation required.
- Dependent’s Gender: Update your dependent’s gender in My Insurance Benefits. To make changes log into Workday (myworkday.wisconsin.edu). Once in Workday, from the Menu in the left go to Personal, then select Benefits and Pay. In the Suggested Links, select My Benefits to access the benefits enrollment system, My Insurance Benefits.
When you update your personal information most benefit vendors are notified of the changes automatically.
After you make the update with the Universities of Wisconsin and it has been processed, the plan administrators for the following benefit plans will automatically be notified:
- State Group Health Insurance
- Dental Insurance
- Vision Insurance
- State Group Life Insurance
- Individual & Family Life Insurance
- Accidental Death & Dismemberment Insurance
- Accident Insurance
- Flexible Spending Account(s)
- Health Savings Account
- Income Continuation Insurance
- Wisconsin Retirement System (WRS)
- Wisconsin Deferred Compensation (WDC) 457 Program
Complete your address change with your 403(b) recordkeeper over the phone or in writing. TIAA permits online address changes.
Name changes must be made in writing; contact your recordkeeper to obtain a form.
How Benefits are Affected
If you have an address change you can make certain benefits changes within 30 days of the move. Information on changes you can make are listed below.
To review which plans you are currently enrolled log into Workday (myworkday.wisconsin.edu). Once in Workday, from the Menu in the left go to Personal, then select Benefits and Pay. In the Suggested Links, select My Benefits to access the benefits enrollment system, My Insurance Benefits.
You may change health insurance plans and/or health insurance carriers within 30 days of your address change. The change is effective the first of the month on or following receipt of your application.
To make a change log into Workday (myworkday.wisconsin.edu). Once in Workday, from the Menu in the upper left go to Personal, then select Benefits and Pay. In the Suggested Links, select My Insurance Benefits to access the benefits enrollment system. For assistance with determining the correct date to use when entering your life event in My Insurance Benefits review Effective Dates of Qualifying Life Events (ET-1142a).
- If you change health insurance plan carriers (for example, GHC, Dean, Quartz, etc.), your annual deductible and medical out-of-pocket limit will reset. However, your prescription drug annual out-of-pocket limit will not reset.
- If you change to or from an HDHP and:
- Change health insurance plan carriers, all limits will reset (for example, non-HDHP with GHC to HDHP with Dean or HDHP with GHC to non-HDHP with Dean).
- Stay with the same health insurance plan carrier, your limits will not reset. (for example, non-HDHP with GHC to HDHP with GHC)
After you update your name and/or address with the Universities of Wisconsin, the FSA administrator will automatically be notified.
Health Care FSA
No impact. You may not make any changes to your Health Care FSA election.
Dependent Day Care FSA
You may change your Dependent Day Care FSA election within 30 days of your move if the change in residence results in a change to the cost of your Dependent Day Care.
To change your election log into Workday (myworkday.wisconsin.edu). Once in Workday, from the Menu in the upper left go to Personal, then select Benefits and Pay. In the Suggested Links, select My Insurance Benefits to access the benefits enrollment system.
