The Department of Employee Trust Funds (ETF) wants to know about your preferences for paying for health insurance. Please complete the Health Insurance Program Plan Design Survey by Monday, December 1, 2025.

The survey is intended for benefits eligible employees.

The short survey collects demographics information and preferences for premium rates and other cost sharing (for example, copays, deductibles, coinsurance, out-of-pocket limits) for health and prescription benefits. Responses are confidential. Only combined and anonymous results will be reported.

Background

ETF and the Group Insurance Board (Board) periodically review the health insurance plan design structures to maintain affordability and long-term sustainability. The plan design includes what’s covered, how much you pay for care, and how costs are shared between you and the plan.

Feedback is highly valued by the Board when making decisions about program changes.

ETF will analyze survey results and aggregate claims utilization data to make recommendations to the Board about possible plan design changes in the 2027 plan year and beyond.
