Important Health Coverage Tax Documents

Form 1095-C for the 2025 tax year is available upon request.

Due to a recent change in federal law (the Paperwork Burden Reduction Act), the Universities of Wisconsin is no longer required to automatically mail a copy of the Employer-Provided Health Insurance Offer of Coverage (Form 1095-C) to every employee. However, we will provide the form to any individual who requests one.

Current Employees: Form 1095-C will be available in Workday in January of 2026. Once available, you can view or print a copy of the form from Workday.

Former Employees: To request a copy of your Form 1095-C, contact UW-Shared Services through one of the following methods:

If you request a copy, it will be provided to you no later than 30 days after we receive your request or by the furnishing deadline (typically early March of the following year), whichever is later. Electronic delivery is available if you have consented to receive communications electronically.

Note:

  • This notice will remain accessible on this website through October 15 of the year following the applicable calendar year.
  • Form 1095-C is an informational return about the health insurance coverage offered by your employer, the Universities of Wisconsin. You typically do not need this form to file your federal income tax return, but you may want a copy for your records.
  • The Universities of Wisconsin remains obligated to file all Forms 1095-C and the associated Form 1094-C with the IRS.
