Request Phone for Employee

Phone Request - New

Supervisors may use this form to request phone setup for a new employee. Please submit this form at least one week in advance of your new employee's start date.
  • Enter your e-mail address
  • Please submit this form at least one week in advance of your new employee's start date
    Date Format: MM slash DD slash YYYY
  • Personnel Action Form Completed * Required
    A Personnel Action Form must be completed and submitted for all new employees. An employee will not be issued a network logon or e-mail address until this form has been received by Human Resources. Contact your Human Resources representative for more information.
  • Employee Building * Required
    Please select the work location of your new employee
  • Enter the room number or location within the selected building where the employee will be located
  • Phone Available * Required
    Indicate whether a Cisco phone is already installed at the employee's workstation
  • Enter the number assigned to the existing phone if known
  • Retain Phone Number
    Indicate whether the new employee must keep the number already assigned to this phone. This is only recommended if the number is tied to the employee's position (e.g. program assistants, executive assistants, department contacts, etc.).
