Public Service Loan Forgiveness (PSLF) Program

Introduction

The Universities of Wisconsin is a qualified employer for the Public Service Loan Forgiveness (PSLF) and Temporary Expanded Public Service Loan Forgiveness (TEPSLF) programs.

The UW-Shared Services (UWSS) manages employment verifications for the PSLF program for all UW universities.

Instructions

There are multiple ways to submit your PSLF employment verification to UWSS. We recommend the Federal Student Aid prefers the PSLF Help Tool for the most efficient verification. If you are unable to submit the verification using the online PSLF Help Tool, instructions for sending via email, fax, or drop-off.

PSLF Help Tool (DocuSign)

  1. If using Docusign, provide the following email address for the Official’s email: UWSSAdminSup@uwss.wisconsin.edu.
  2. Ensure the following fields are accurate before submitting:
    • Employer name: University of Wisconsin System
    • FEIN: 396006492
    • Employer address: 660 W. Washington Avenue, Suite 201, Madison, WI 53703
  3. If any fields other than the dates need to be edited, UWSS is unable to sign and will email you to explain which fields must be updated to resubmit your employment verification.
  4. If you are unsure of your dates of employment or average hours worked, you can send an email to GetHelpUW to obtain the information before submitting your PSLF employment verification.

Federal Student Aid prefers the PSLF Help Tool. For those who are not able to submit their verification using the online PSLF Help Tool, we’ve provided instructions for sending via email, fax, mail, or drop-off.

Email

  1. Obtain the federal PSLF/TEPSLF form found on the studentaid.gov website.
  2. You can complete Section 4 or submit to UWSS for completion. When completing Section 4, use the following information:
    • Employer name: University of Wisconsin System
    • FEIN: 396006492
    • Employer address: 660 W. Washington Avenue, Suite 201, Madison, WI 53703
  3. You MUST REMOVE the entire SSN from the attachment and body of the email. Including page 1 and the tops of page 2 and 3 of the PSLF application. Any attachments sent with your SSN on either page or in the body of the email will be deleted.

Email the form to GetHelpUW with the following Subject Line: [First Name] [Last Name] – EMPLID: [Your 8-Digit EMPLID] – PSLF Request. Note: If you are unsure your UWs employee ID number (EMPLID), you can find it on your payslip or within your profile in Workday. If you are unable to location your EMPLID, you will need to submit your PSLF application via fax, mail, or drop off (see below).

UWSS is not able to to forward emailed documents to the U.S. Department of Education on your behalf. UWSS will verify, sign, and return your documents to the email sent from. Send your employment verification to PO Box 300010, Greenville, TX 75403 for FAX (540) 212-2415.

Fax / Mail / Drop-Off

  1. Complete and print pages 1-3 of the federal PSLF/TEPSLF form found on the Federal StudentAid website
  2. Sections 1-3 (pages 1-2) must be completed, signed, and dated by the employee.
  3. When completing Section 4, use the following information:
    • Employer name: University of Wisconsin System
    • FEIN: 396006492
    • Employer address: 660 W. Washington Avenue, Suite 201, Madison, WI 53703
  4. Complete and print the UWSS cover sheet.
    • Include full name, phone number, and preferred email address.
    • If known, include your 8-digit UWs EmplID (this number is often different from your university ID.) If not known, leave blank.
    • Select one of the following two options:
      • You would like UWSS to submit the completed form to the U.S. Department of Education on your behalf. You will receive a copy along with a fax confirmation with the date/time the paperwork was sent. Be sure to keep a copy of the completed PSLF to confirm your application was processed correctly.
      • You would like to submit the form to the U.S. Department of Education once verified and signed.
    • Indicate your personal mailing address or fax number for us to return the original. UWSS is unable to send send completed forms with a SSN via email.
    • Include any notes about your employment history that may help our team (e.g., I would like you to verify part-time employment as well as full-time employment).
  5. Submit the cover sheet plus pages 1-3 of the PSLF/TEPSLF form to UWSS using one of the following methods. The application is 16 pages, however, UWSS does not need the entire document.

FAQs

  • All universities are within the Universities of Wisconsin. If an employee transfers from one campus to another without a gap in employment, only one verification form is required.
  • If there is a gap in employment, separate forms must be completed for each of the time periods.
  • Part-time hours and full-time hours must be indicated on separate PSLF/TEPSLF forms.
  • Hours Per Week are typically calculated by the FTE number provided by HR staff: 0.5 FTE = 20 hours; 1.0 FTE = 40 hours (the exception is for LTE and Student positions – see next point). If an employee works two 0.5 FTE positions at the same time, it counts as 1.0 FTE (40 hours/week).
  • For some positions (i.e., student help, LTE, etc.), hours worked per week will also be verified through payroll by determining how many hours per paycheck were recorded then averaged for the length of each position. If the total sum of average weekly hours is less than 30, the position will be indicated as part-time. Average hours totaling more than 30 hours will be indicated as full-time.
  • Hours worked cannot be verified for positions that were paid via lump sum unless the employee can provide a copy of their appointment letter or contract indicating the specific position will be full-time (30+ hours/week).
  • If an you disagree with the information verified by the UWSS office, we recommend you confirm the HRS Job Employment data with their local HR Office.
  • Always save the completed PSLF paperwork and confirm that the U.S. Department of Education has processed it correctly.
  • Any questions not addressed above can be sent to GetHelpUW for review. Never attach PSLF paperwork with SSNs to an email.
