W-2 Forms
This form is the most commonly applicable form for taxable income reporting purposes through regular employment with the University of Wisconsin.
1042-S Requests
This form only apply to people who:
- had a tax treaty exemption applied to their incomes OR
- received a scholarship or fellowship through payroll before passing the IRS substantial presence test
Before requesting a duplicate 1042-S, make sure you were eligible to receive one.
Fellowship / Scholarship Letter
Reports non-service scholarships / fellowships paid to resident aliens, permanent residents or US persons. For Resident Aliens and/or Permanent Residents, each dollar of income is only reports on one form / letter based on your residency status.
1098-T (Tuition Statement) – Not provided by UW-Shared Services
Please contact your campus bursar’s office for a copy of this form.
