Occasionally, the Universities of Wisconsin may request additional information after filling out the application for reciprocity benefits. Upon request, we would ask you to fill out one of the following forms.

Student Residence Verification Form

Purpose: This form is used to verify the applicant’s physical presence and intent to establish and maintain a permanent home in Wisconsin.

Key Information Collected:

Filing of Wisconsin state income tax returns: Attestation that applicant has filed state income tax returns in Wisconsin
Duration of residence: How long the applicant has lived in Wisconsin.
Employment details: Information about the applicant’s employer and duration of employment.
Citizenship details: Upload documentation on visa status.

Student Residence Verification Form

Parent/Guardian Residence Verification Form

Purpose: This form is used to verify the residence status of the applicant’s parent(s) or legal guardian(s) if the applicant is a minor or a dependent.

Key Information Collected:

Filing of Wisconsin state income tax returns: Attestation that the parent/guardian has filed state income tax returns in Wisconsin
Duration of residence: How long the parent/guardian has lived in Wisconsin.
Employment details: Information about the parent/guardian’s employer and duration of employment.
Citizenship details: Upload documentation on visa status.

Parent/Guardian Residence Verification Form

Student Dependency Verification Form

Purpose: This form is used to verify the dependency status of the applicant and the financial support provided by the parent(s) or legal guardian(s).

Key Information Collected:

Proof of dependency: A signed affidavit from the parent/guardian.
Details of financial support: Information about the percentage of support and duration provided by the parent/guardian.
