Use this form to submit a change request (not a new tool request) the DLE Service Management Team.

Request Submitter Name * Required
Name of the individual requesting the change
Requester Name * Required
Name of the DLE Institution Liaison or designee submitting the request. The submitter acknowledges that the request has been reviewed and approved by the institution.
