employment you may be eligible to continue or convert your benefits.<\/p>\n<h2>How Benefits are Affected<\/h2>\n<p>Below is information on how each benefit plan is affected when terminating your employment terminates. You may be eligible to either convert or continue coverage. If you convert coverage to an individual policy, the coverage is more expensive. If you continue coverage, you pay the same premium as active employees.<\/p><p>You are eligible to <strong>convert<\/strong> your coverage to an individual policy if you:<\/p><ul><li>Are under age 65; and<\/li><li>Were insured for the entire six months prior to your termination.<\/li><\/ul><p>You may convert coverage to an individual whole life insurance policy known as Secure Protector Whole Life. To do so, submit a conversion application, and premium payment, within 31 days of the coverage end date.<\/p><p>You are eligible to <strong>continue<\/strong> your group policy if:<\/p><ul><li>Your Wisconsin Retirement System (WRS) coverage began before 1-1-90 or you have been covered under the group life insurance plan for a part of at least 5 calendar years; AND<\/li><li>You meet all the requirements for receiving an immediate WRS annuity except the filing of the application; or<\/li><li>The sum of your creditable years of WRS service before 1-1-90 plus the number of years you have participated in the life insurance plan after 1989 equals 20 years; or<\/li><li>You have 20 years of WRS service on payroll with the State of Wisconsin.<\/li><\/ul><p>You may continue coverage by submitting an application within 31 days of your coverage end date.<\/p><p>Review <a href=\"https:\/\/etf.wi.gov\/publications\/et4104\/direct\" target=\"_blank\" rel=\"noopener\">State Group Life Insurance After You Terminate Employment (ET-4104)<\/a> for more information.<\/p>                <\/div>\n    <\/div>\n<\/div>\n            <div class=\"panel panel-default \" id=\"panel_individual-family-life-insurance\">\n    <div class=\"panel-heading\" id=\"heading_individual-family-life-insurance\">\n        <h4 class=\"panel-title\"><button href=\"#individual-family-life-insurance\" data-toggle=\"collapse\" class=\"collapsed panel-link\" aria-expanded=\"false\" aria-controls=\"individual-family-life-insurance\">Individual &amp; Family Life Insurance<\/button><\/h4>    <\/div>\n    <div id=\"individual-family-life-insurance\" class=\"panel-collapse collapse\"  role=\"tabpanel\" aria-labelledby=\"heading_individual-family-life-insurance\" role=\"region\">\n        <div class=\"panel-body copy\">            \n            \n        <p>Coverage ends at the end of the month in which your employment terminates.<\/p><p>You may convert coverage to an individual whole life insurance policy. This must be done within 60 days of your coverage end date (reference UW System Policy # GTU-8364005).<\/p>                <\/div>\n    <\/div>\n<\/div>\n            <div class=\"panel panel-default \" id=\"panel_accident-insurance\">\n    <div class=\"panel-heading\" id=\"heading_accident-insurance\">\n        <h4 class=\"panel-title\"><button href=\"#accident-insurance\" data-toggle=\"collapse\" class=\"collapsed panel-link\" aria-expanded=\"false\" aria-controls=\"accident-insurance\">Accident Insurance<\/button><\/h4>    <\/div>\n    <div id=\"accident-insurance\" class=\"panel-collapse collapse\"  role=\"tabpanel\" aria-labelledby=\"heading_accident-insurance\" role=\"region\">\n        <div class=\"panel-body copy\">            \n            \n        <p>Coverage ends at the end of the month in which your employment terminates.<\/p><p>You may port coverage by submitting a portability application. This must be done within 31 days of the coverage end date. You may continue coverage until age 70.<\/p><p>Shortly after your last day you will receive a continuation notice and portability form from Securian.\u00a0The portability form can also be accessed on <a href=\"https:\/\/web1.lifebenefits.com\/content\/lifebenefits\/wietf\/en.html\" target=\"_blank\" rel=\"noopener\">Securian's website<\/a> under Forms &amp; Documents.\u00a0<\/p>                <\/div>\n    <\/div>\n<\/div>\n            <div class=\"panel panel-default \" id=\"panel_income-continuation-insurance\">\n    <div class=\"panel-heading\" id=\"heading_income-continuation-insurance\">\n        <h4 class=\"panel-title\"><button href=\"#income-continuation-insurance\" data-toggle=\"collapse\" class=\"collapsed panel-link\" aria-expanded=\"false\" aria-controls=\"income-continuation-insurance\">Income Continuation Insurance<\/button><\/h4>    <\/div>\n    <div id=\"income-continuation-insurance\" class=\"panel-collapse collapse\"  role=\"tabpanel\" aria-labelledby=\"heading_income-continuation-insurance\" role=\"region\">\n        <div class=\"panel-body copy\">            \n            \n        <p>Coverage ends on the date in which your employment terminates. There is no option to continue or convert coverage.<\/p>                <\/div>\n    <\/div>\n<\/div>\n            <div class=\"panel panel-default \" id=\"panel_flexible-spending-account\">\n    <div class=\"panel-heading\" id=\"heading_flexible-spending-account\">\n        <h4 class=\"panel-title\"><button href=\"#flexible-spending-account\" data-toggle=\"collapse\" class=\"collapsed panel-link\" aria-expanded=\"false\" aria-controls=\"flexible-spending-account\">Flexible Spending Account<\/button><\/h4>    <\/div>\n    <div id=\"flexible-spending-account\" class=\"panel-collapse collapse\"  role=\"tabpanel\" aria-labelledby=\"heading_flexible-spending-account\" role=\"region\">\n        <div class=\"panel-body copy\">            \n            \n        <h4>Health Care FSA<\/h4><p>At the end of your employment, you have the following options:<\/p><ul><li>You may continue coverage by making after-tax contributions directly to the Department of Employee Trust Funds (ETF). If you continue your coverage, you may incur eligible expenses until the last day of the month in which your final contribution is made to ETF or December 31, whichever is earlier.<\/li><li>If you do not continue your coverage by making after-tax contributions directly to ETF, you may only incur eligible expenses through the end of the month in which your employment ends.*<\/li><li>If you only have carryover funds from a prior plan year, meaning you are not contributing in the current plan year, eligible expenses must be incurred by the last day of the month in which your employment ends.<\/li><\/ul><p><em>*Applies to carryover and current plan year funds.<\/em><\/p><p>Note: Your payment card will be deactivated at the end of the month in which your employment ends. If you have eligible expenses to submit for reimbursement after your payment card is deactivated, you will need to pay out-of-pocket, then submit your claims manually (either electronically or via U.S. Mail).<\/p><h4>Dependent Day Care<\/h4><p>At the end of your employment, you cannot continue to make Dependent Day Care Account contributions. However, you can continue to incur expenses and request reimbursement until you exhaust your account balance or the end of the plan year (December 31), whichever is earlier.
