The plan provides a cash payment to help pay out-of-pocket expenses in the event of an accident. The plan also includes an accidental death &amp; dismemberment benefit.<\/p>\n<p>In addition to benefits payable for accidents, this plan provides the following:<\/p>\n<ul>\n<li>Benefits for covered injuries including support care.<\/li>\n<li>More than one benefit payment may be payable for a single covered accident.<\/li>\n<li>Identity theft services from Generali Global Assistance are available to employees who enroll in Accident Insurance. Services include identity theft education and personalized resolution resources. See the Identity Theft flyer in the Forms &amp; Resources section below for additional details.<\/li>\n<li>Benefits for accidental death and dismemberment.<\/li>\n<li>Access to Empathy through Securian Financial. This program provides comprehensive support to assist beneficiaries following the loss of a loved one or for a claim involving a life-threatening diagnoses. The program is offered free of charge to employees enrolled in Accident Insurance. For more information review the Empathy Flyer in the Forms &amp; Resources section below.<\/li>\n<\/ul>\n<p>The Accident Insurance plan (Group Policy #76038) is administered by the <a href=\"https:\/\/etf.wi.gov\/\" target=\"_blank\" rel=\"noopener noreferrer\">Department of Employee Trust Funds (ETF)<\/a> and underwritten by Securian Financial Group.<\/p>\n<h2>Eligibility<\/h2>\n<p>You must be eligible for State Group Health Insurance as an active employee to be eligible for this plan.<\/p>\n<p>Note: If you and your spouse are both eligible for this plan as Universities of Wisconsin employees or as a Universities of Wisconsin and State of Wisconsin employee, you and your spouse may want to each enroll for employee coverage to receive the maximum accidental death &amp; dismemberment benefit available under the plan. You and your spouse may be covered as an employee OR a dependent \u2013 not both. This rule also applies to your eligible children; they may be covered as dependents on one plan &#8211; not both.<\/p>\n<h2>Enrollment<\/h2>\n<p>You have 30 days from your date of employment or your newly benefits-eligible job to enroll in Accident Insurance. Coverage will begin the first of the month on or following your date of employment or date of obtaining a benefits-eligible job.<\/p>\n<p>If you do not enroll in Accident Insurance during your initial 30-day enrollment period, you may enroll in the fall during the benefits enrollment period with coverage effective for the upcoming calendar year or if you have a qualifying life event (see <a href=\"https:\/\/www.wisconsin.edu\/ohrwd\/benefits\/life-events\/\" target=\"_blank\" rel=\"noopener noreferrer\">Life Events<\/a>).<\/p>\n<h2>Coverage Levels<\/h2>\n<p>Accident Insurance provides a cash payment directly to you in the event of an accident. The payout depends on the injury suffered, the care you receive and the terms and conditions of the policy. You (not your providers) file claims directly with Securian.<\/p>\n<h2><strong>Continuation<\/strong><\/h2>\n<p>Continuation of group policy coverage at the end of employment is available. To keep your coverage submit a portability election form within 31 days after receipt of notification of portability.<\/p>\n<p>Shortly after your coverage ends you will receive a continuation notice and portability form from Securian. The portability form can also be accessed on <a href=\"https:\/\/web1.lifebenefits.com\/content\/lifebenefits\/wietf\/en.html\" target=\"_blank\" rel=\"noopener\">Securian&#8217;s website<\/a>\u00a0under Forms &amp; Documents.<\/p>\n<h2>Beneficiary Information<\/h2>\n<p>You are encouraged to designate a beneficiary to ensure benefits are paid according to your wishes. Whenever a life event, such as a marriage, divorce, or birth of a child, occurs, review and updated your beneficiary designations.
