    2008 Programs of Distinction Award Application

     Application Deadline: February 15, 2008

    Directions: A complete application will include all of the information detailed in sections I-III, as well as a letter of recommendation. Electronic application is highly encouraged, although paper copy will be accepted. All materials must be received electronically or postmarked on or before February 15, 2008.

    II Collaborative Partners

    List all partners that play an active role in your program (i.e., individuals/organizations involved in the design and/or implementation of program); include names/titles of key representatives and organization represented.

    III Narrative Program Description

    Please provide a brief summary of your program. Be sure to address the following areas in your summary (three pages maximum, double spaced, 12 point font, 1 inch margins):

    1. Program Outcomes: List those designed to improve student learning and/or enhance quality teacher preparation;
    2. Key Program Activities:. State how activities are based on relevant research, could be replicated in other settings, are innovative, reflect an appropriate use of technology, help further program goals, and are responsive to issues of diversity; and
    3. Methods of Assessment: Describe how program outcomes have been assessed or describe your plans for assessment and, if applicable, provide a summary of the evidence that demonstrates that outcomes have been achieved.

    IV Letter of Recommendation

    Please forward a one-page letter of recommendation from someone familiar with your program.

    V Supplemental Materials

    If you included supplemental materials, please limit your materials to two pages maximum. For example, you may forward sample artifacts that directly support the description of your program and/or demonstrate outcome attainment (e.g., program brochures, samples of program products, summary of evidence in support of student achievement/teacher quality, etc.). Please do not include additional letters of recommendation as supplemental materials.
