Controlled Substance Special Use Authorization
Obtaining Authorization to Possess Controlled Substances
In accordance with federal and state laws, it may be possible to obtain a Special Use Authorization (SUA) to possess controlled substances for research, analytical laboratory, industrial/commercial processing, instructional and other activities
You will need approval from both the State of Wisconsin Department of Regulation and Licensing (DRL) and the U.S. Drug Enforcement Administration (DEA).
Forms you will need to file:
Wisconsin (Department of Regulation & Licensing)
Wisconsin Form 2275 - Instructions for Form 2184, the WI APPLICATION FOR CONTROLLED SUBSTANCES SPECIAL USE AUTHORIZATION (below).
Wisconsin Form 2277 describes the security requirements.
Wisconsin Form 2184 is the WI APPLICATION FOR CONTROLLED SUBSTANCES SPECIAL USE AUTHORIZATION
Note no fee is charged for permits issued to employees of state agencies or institutions.

Federal (Drug Enforcement Administration)
Form 225 is the federal (DEA) application. It can be filled out online at:

Fee will be $184.00
You may file forms simultaneously. If you do so, write "Pending" in the State Controlled Substance License Number space in section 4 of the federal form (225).
